Job summary

Comagine Health in the United States is seeking a Home and Community Based Coordinator to support timely discharges, care transition services, and collaboration with providers and community partners.

You will conduct needs assessments, develop service plans, maintain resources, and assist Medicaid consumers during transitions, while coordinating data entry and filing to ensure seamless care journeys.

Qualifications

• State knowledge of Oregon Home and Community Based Services (preferred).
• Proficient with MS Office and databases; strong organizational skills.
• Excellent oral and written communication and interpersonal abilities.

Responsibilities

• Facilitate timely discharges and care transition support with providers and community partners.
• Conduct brief functional needs assessments and develop service plans.
• Provide support to Comagine Health staff and Medicaid consumers during transitions.
