Job summary

Qualifications

Responsibilities

• Follows a 30-day transition program from hospital admission through discharge to other settings.
• Collaborates with discharge planners, hospitalists, providers and family to ensure safe transitions.
• Educates members on medications, follow-up care, and signs of worsening conditions.
• Coordinates interdisciplinary care team meetings and monitors care needs post-discharge.
