Job summary

Texas Health Resources is seeking a Care Transition Manager RN on a PRN basis at the Fort Worth hospital. The role involves discharge planning, care coordination, and clinical decision-making to support patient transitions.

Qualifications

• BSN required; ADN grandfathered for some hires
• 3 years staff nurse at an acute care hospital required
• 1 year discharge planning/care management preferred
• RN – Registered Nurse required on hire
• CPR on hire required
• ACM/ CCM/ ANCC preferred on hire
