Texas Health Resources invites applications for a Care Transition Manager Social Worker at our Ft. Worth facility. The role supports hospital discharge planning and care transitions, requiring an MSW and LMSW/LCSW eligibility.

The position is 24 hours per week on weekends (Sat/Sun, 7:00 AM–7:30 PM). Ideal candidates will have hospital social work experience, strong communication, and collaborative skills to coordinate with the care team and community resources.

Qualifications

• Master's Degree in Social Work required.
• 3+ years hospital/medical social work preferred.
• 1 year discharge planning/care management preferred.

Responsibilities

• Engage in complex clinical decision-making and discharge planning.
• Provide psychosocial evaluation and crisis intervention.
• Coordinate referrals to community resources and transitions to appropriate care levels.
• Collaborate with interdisciplinary team to ensure timely patient transitions.
