Job summary

Corewell Health is seeking a part-time Care Management/Discharge Planning RN in Dearborn, MI. The role focuses on integrating care coordination, utilization management and discharge planning to optimize patient outcomes and resource use.

The position requires a Bachelor’s in Nursing, MI RN license, and 2 years of relevant experience; a BSN is preferred or pursued within 2 years. Hours are 24 per week, daytime shifts. Benefits are offered with the site.

Qualifications

• Bachelor's degree in nursing required.
• Will consider non-BSN RN if pursuing BSN with completion within 2 years of hire.
• 2 years of relevant experience minimum; 3 years preferred in care management, utilization review, discharge planning.

Responsibilities

• Integrates cost, quality and utilization to facilitate admission, ongoing stay and discharge of patients.
• Reviews appropriateness of admission/continued stay based on medical necessity.
• Coordinate care, discharge planning, and transitions for hospitalized patients to ensure quality and cost-effectiveness.
• Collaborate with physicians, payers and care teams to obtain authorization for care and appropriate reimbursement.
• Communicate with payers, patients and families to coordinate care and transitions.
