Triple-S Management Corp. in Puerto Rico seeks a registered nurse to guide Alianza CarePoint members through the transition of care, ensuring safe discharge and continuity of services.

The role requires a BSN, current Puerto Rico RN license, and 1–3 years in clinical or managed care settings; you will liaise with hospitals, PCPs, and care teams to reduce readmissions and support members and caregivers.

Qualifications

• Current RN license in Puerto Rico without restrictions.
• BSN or higher required with active nursing license in Puerto Rico.
• 1–3 years of clinical experience and managed care exposure.

Responsabilités

• Identifies discharged members from the discharge census.
• Contacts the member to complete assessments to guarantee continuity of care and wellbeing.
• Coordinates the PCP 7 days post discharge appointment.
• Maintains effective communication with the member during the 30 days post discharge period.
• Assists with necessary preauthorizations when required.
