Job summary

Triple-S in Puerto Rico seeks a Registered Nurse to support coordinated care, strengthen partnerships with primary care providers, and drive adherence to medical guidelines and HEDIS indicators. This role emphasizes documentation, data review, and collaboration with care teams to improve patient outcomes.

You will participate in patient transitions, on-site or telephonic follow-ups, and reviews of referrals while maintaining HIPAA standards.

Qualifications

• RN license in Puerto Rico (PR) without restrictions.
• 1–3 years of Coordinated Care experience, preferably.
• Bachelor's Degree in Nursing required.

Responsibilities

• Provides complex care planning to high‑risk patients and coordinates transitions of care.
• Performs follow‑up telephonic care calls to patients and families.
• Reviews PCP referrals to determine program and service needs.
• Documents care rendered and all communications in appropriate databases.
• Implements care plans and follows HIPAA standards.
• Collaborates with PCP, Care Point Medical Director, Pharmacist, and team members to ensure care goals.
