Job summary

Hillsboro Medical Center seeks a Care Management Discharge Coordinator to join the interdisciplinary team. You will help create safe discharge plans, coordinate post-acute care, and ensure resources and insurance coverage are in place for a smooth transition from hospital.

This role requires collaboration with physicians, nurses, and payors, plus documenting discharge plans in the EMR and arranging follow-up appointments.

Qualifications

• Two years healthcare delivery system related experience.
• Discharge planning, utilization review, case management or healthcare insurance experience preferred.
• Current Oregon license as LPN, CNA II or Medical Assistant.

Responsibilities

• Aid Case Management team with discharge planning and post-acute care.
• Coordinate communication with physicians, clinics, community resources and patients.
• Provide timely information to payors and staff.
• Assist in securing post-discharge services and transportation.
• Document discharge planning in EMR and arrange follow-up appointments.
