Mercy Hospital Jefferson in Festus, MO is seeking a Care Management Associate to support the Care Management team in transition planning and length of stay management. The role focuses on coordinating information and communication and performing clerical duties aligned with Mercy service standards.

You will collaborate with healthcare professionals to ensure patients receive the right information at the right time, maintain confidentiality, and help optimize patient flow in a fast-paced setting.

Qualifications

• Education: High School Diploma required.
• Experience: 1 year healthcare experience preferred.
• Certification/Registration: BLS at hire date or within 90 days of hire.

Responsabilités

• Coordinate information and communication for the Care Management team.
• Support transition planning and length of stay management.
• Perform clerical duties to aid the Care Management team.
• Ensure information is shared accurately and in a timely manner.
