Elara Caring is seeking a CareTeamSupervisor in Medford, NY for a full-time, on-site schedule (Mon-Fri, 8:30 AM–5:00 PM) with a pay range of $22–$24 per hour. You will coordinate personal care services, maintain client records, and oversee caregiver scheduling, ensuring compliance with licensure and payer requirements.

The role includes coaching caregivers, monitoring service delivery, and supporting branch growth.

Qualifications

  • High School Diploma or GED required.
  • Associate or bachelor’s degree preferred.
  • Experience working with elderly or disabled populations preferred.
  • Five years of experience in a fast-paced office or healthcare environment preferred.
  • Working knowledge of Medicare/Medicaid and homecare operations preferred.
  • Strong computer proficiency and documentation skills.
  • Excellent organizational and multitasking abilities.
  • Professional verbal and written communication skills.
  • Strong leadership, coaching, and motivational skills.
  • Ability to thrive in a fast-paced, high-volume environment.
  • Bilingual fluency may be required based on location.

Responsibilities

  • Coordinate and schedule personal care services, including coverage for planned and unplanned absences.
  • Maintain accurate client service records, plans of care, caregiver schedules, and required documentation.
  • Review and manage service authorizations and contract-specific requirements.
  • Monitor reports to ensure accuracy in billing, payroll, utilization, and service delivery.
  • Support branch growth initiatives and operational planning efforts.
  • Ensure ongoing compliance with all licensures, payer, contract, and regulatory standards.
  • Provide guidance, coaching, and support to caregivers to promote performance and engagement.
  • Ensure caregiver compliance with required trainings, certifications, health assessments, and in-services.
  • Conduct caregiver orientations as required.
  • Collaborate with payroll and timekeeping teams.
  • Monitor daily service delivery to ensure adequate caregiver availability and coverage.
  • Monitor systems to confirm authorizations align with scheduled services and review utilization reports.
  • Communicate professionally and effectively with caregivers, clients, case managers, and leadership.
  • Represent Elara Caring in a professional and positive manner within the community.
  • Perform additional duties and projects as assigned to support team and organizational success.

As the CareTeamSupervisor , you’ll contribute to our success in the following ways:

What is Required?

You will report toCareTeamManager, Lead, Branch Director, or Regional Branch Director.
