Job summary

DENBIGHSHIRE COUNTY COUNCIL is seeking a compassionate Senior Care & Support Worker to join the Independence at Home Team. You will lead a team of Care & Support Workers, design person-centred reablement plans, and supervise day-to-day operations to promote independence and wellbeing for vulnerable adults in their own homes.

You will coordinate care using digital systems like PARIS and iTrent, contribute to recruitment and induction, and work with internal and partner teams to maintain
