Job summary

Centerstone is seeking a dedicated professional to provide person-centered assistance to clients, coordinating care with a multidisciplinary team and connecting clients to community resources. You will deliver crisis intervention and support to families, following CCBHC standards and organizational protocols.

The role emphasizes building life skills, documentation, and collaboration with schools and service providers to ensure comprehensive care for youth and families.

Qualifications

• Requires Associate's degree; two years of related field experience may substitute for degree.
• Two years professional experience working with youth and families preferred.

Responsibilities

• Coordinates client care with multidisciplinary team to develop and maintain clinical records.
• Teaches activities of daily living in group and individual settings per guidelines.
• Connects clients with community resources and communicates with schools and service providers.
• Provides on-site and telephone crisis intervention and follow-up to clients and families.
• Understands Centerstone's Certified Community Behavioral Health Clinics (CCBHC) model.
• Performs duties in accordance with CCBHC standards.
