Job summary

Centerstone seeks a dedicated professional to provide life skills development and crisis intervention for clients. You will coordinate care with a multidisciplinary team and help connect clients to community resources, schools, and family service providers.

This role emphasizes documenting clinical records, teaching daily living activities, and ensuring compliance with CCBHC standards while delivering compassionate, client-centered care.

Qualifications

• Associate's degree required; two years of experience may substitute for degree.
• Two years of professional experience working directly with youth and families preferred.
• N/A Certification/Licensure.

Responsibilities

• Coordinate client care with a multidisciplinary team and maintain clinical records.
• Teach activities of daily living in group and individual settings; ensure documentation.
• Connect clients with community resources; communicate with schools and family service providers.
• Provide telephone and on-site crisis intervention and follow-up.
• Follow CCBHC standards and organizational guidelines in all duties.
