Job summary

Centerstone seeks a dedicated professional to provide life skills development and crisis intervention for clients. You will coordinate care with a multidisciplinary team and help connect clients to community resources, schools, and family service providers.

This role emphasizes documenting clinical records, teaching daily living activities, and ensuring compliance with CCBHC standards while delivering compassionate, client-centered care.

Qualifications

• Associate's degree required; two years of experience may substitute for degree.
• Two years of professional experience working directly with youth and families preferred.
• N/A Certification/Licensure.

Responsibilities

• Coordinate client care with a multidisciplinary team and maintain clinical records.
• Teach activities of daily living in group and individual settings; ensure documentation.
• Connect clients with community resources; communicate with schools and family service providers.
• Provide telephone and on-site crisis intervention and follow-up.
• Follow CCBHC standards and organizational guidelines in all duties.

Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their familes.ESSENTIAL DUTIES & RESPONSIBILITIESCoordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards.Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family participates with or is receiving services from.Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.Perform all required job duties and responsibilities in accordance to CCBHC Standards.KNOWLEDGE, SKILLS & ABILITIESEffectively communicate via written, verbal, in person and virtual methods.Customer service skills.Time management skills.Analytical skills.Knowledge of community resources.QUALIFICATIONSEducation LevelAssociate's degree required.
