Location: Chicago IL or Downers Grove IL - Hybrid,

Duration: 3+ months contract to hire

Pay rate: $40.00 to $43.00 /hr or $80k to $87k conversion salary

BASIC FUNCTION:

This position is responsible for discharge care coordination, episodic case management, and pre-admission/post-discharge counseling for an acute condition. The role involves independent duties and assisting other staff, establishing relationships with members during the post-discharge period, providing education and resources, and determining case complexity for referrals.

ESSENTIAL FUNCTIONS:
  1. Discharge care coordination, episodic case management, pre-admission/post-discharge counseling, and identifying treatment options. Collaborate with physicians, discharge planners, and other resources to minimize re-admissions and ER visits.
  2. Support members in the transition of care, including:
  • Consulting with healthcare providers to assess and coordinate care.
  • Personalized outreach based on risk assessments and behaviors.
  • Reconciliation, education, and monitoring medication adherence.
  • Practice within the scope of licensure.
  • Participate as a preceptor for new employee orientation.
  • Maintain licensure and stay updated on regulations through seminars and training.
  • JOB REQUIREMENTS:
    • Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with active license in the state of operation.
    • Minimum 2 years clinical experience.
    • At least 1 year experience in health insurance or managed care.
    • Knowledge of medical management policies.
    • Proficiency with PC and databases.
    • Strong communication and interpersonal skills.
    • Willingness to travel occasionally within the area, possessing a valid driver’s license.
    • For multi-state license positions, additional licenses may be required.

    Required qualifications include LCPC, LCSW, or RN.
