Prairie Vista Village is seeking a dedicated Care Partner (Caregiver) to assist residents with personal care, activities, and meals. The role supports dignity and independence for seniors within a boutique-style community in Altoona, IA.

Qualifications

• 1–2 years of experience caring for the elderly in a residential setting.
• At least 18 years old with a high school diploma or GED.
• Able to follow infection control procedures and provide personal care.

Responsibilities

• Provide personal care to tenants as assigned and per Tenant Service Plans.
• Respond to calls from the emergency call system promptly.
• Assist with daily living activities and group/social recreational activities as requested.
• Maintain a clean, comfortable, and safe living environment for tenants.
• Document changes in tenant needs and health status as required.

The Care Partner or Caregiver position is responsible for providing resident services, including assistance with personal care, medications, social and recreational activities and meal services.Responsibilities:Adheres to and conveys a philosophy that supports the dignity, privacy, independence, choice and individuality of tenants. Demonstrates an appreciation of the heritage, values, and wisdom of the tenant and an understanding of the aging process.Reviews Tenant Service Plans as needed to gain and maintain a familiarity with tenant service needs and preferences.Reviews the Staff 24-Hour Communication log and corresponding Tenant Service Notes on a daily basis for information about changes in tenant service needs and/or health conditions.Provides personal services to tenants as assigned and as indicated on Tenant Service Plans.Responds to calls from the emergency call system in a timely manner.Demonstrates knowledge of and follows infection control procedures.Assists in maintaining a clean, comfortable, and safe environment by providing housekeeping services as needed.Assists with individual tenant and group social/recreational activities as requested; encourages tenant participation in activities.Monitors for changes in tenant needs, preferences, and/or health status, and reports/documents any changes according to established procedures.Responds to emergencies in an appropriate manner as per training received.Works effectively with minimal supervision and in a team setting.Performs all other duties as assigned.A Care Partner is not allowed to perform medication administration without a Assisted Living Medication Manager Certification of completion.Qualifications:Must have 1-2 years’ previous experience working with the elderly, preferably in a residential setting.Must be at least 18 years of age, with a high school diploma or equivalent GED.
