Certified Nursing Assistant / Care Partners
Provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors.
Essential Duties
- Responsibly complete the daily shift report and communicate directly to the next shift any urgent needs.
- Perform all assigned tasks in accordance with established policies and procedures, and as instructed by your supervisors, demonstrating the ability to manage tasks within the scheduled shift.
- Assist with resident care, including housekeeping, laundry, social needs, and demonstrate proper lifting techniques.
- Assist with serving meals and essential clean‑up.
- Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
- Report all changes in the resident’s condition to the DON/Charge Nurse/HWD as soon as practical.
- Agree not to disclose resident’s protected health information and promptly report suspected or known violations to the Administrator.
- Keep certification current and participate in continuing education through staff meetings, in‑service training, and web‑based training.
Education & Experience
- Must possess, as a minimum, a high school education or its equivalent.
- Current certification as a Certified Nurse Assistant from the state.
- Must be a licensed Certified Nurse Assistant who has successfully completed a state‑approved training program and any necessary examinations in accordance with state laws.
- Prefer prior experience in assisted living or skilled nursing units, and prefer at least one year of experience as a certified nurse aide.

- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
- Must have patience, tact, a cheerful disposition, and enthusiasm, and be willing to handle difficult residents.
- Must provide documentation of certification upon application.
