Job summary

Stony Brook Medicine is seeking an Operations and Analytics Manager in Care Management to lead data-driven improvements in transitions of care. You will oversee non-clinical operations, staff coordination, and reporting while partnering with IT to implement QA dashboards and KPI tracking.

The role emphasizes data collection, chart abstraction, and collaboration with clinical teams to enhance performance and reporting for regulatory and internal purposes.

Qualifications

• Bachelor’s Degree with project management skills and the ability to work independently.
• At least 5 years’ experience in the Care Management and Transitions of Care operations and/or analytics.
• 1-2 years’ experience working with grants.
• Intermediate computer skills including spreadsheet and word processing.
• Superior organization skills, detail oriented, and ability to be persistent and follow through.
• Experience with health care quality reporting and/or interpreting health care regulatory/accreditation requirements.
• Experience with data visualization tools including Tableau.
• Strong analytical skills, ability to think critically, effective decision-making/problem solving skills; strong financial, and quantitative skills.

Responsibilities

• Directs and manages non-clinical operations and services including staffing and project management of project coordinator and quality managers.
• Prepares reports to identify training opportunities for departmental divisions and individual staff.
• Identifies and reports workflow deficiencies based on data assessment.
• Supports departmental Quality Assurance (QA) activities.
• Produces QA reports to Care Management leadership (weekly, monthly, quarterly, annual).
• Develops data collection methods using Excel, Access, Crystal Reporting, Cerner DA2, Vizient Tableau.
• Works with clinical staff to enhance clinical data capture and analysis for quality improvement reporting.
• Leads Patient Experience initiatives including flow optimization.
