Job summary

Integrity House in Newark, NJ is seeking a Nurse Care Manager to deliver low-threshold health services at the Integrity Wellness Center. This hands-on role provides assessments, triage, wound care, HIV/HCV testing, vaccination, and linkage to care in a stigma-free setting.

The RN will coordinate referrals, educate on prevention, and support clients with complex health needs in a compassionate, non-judgmental manner.

Qualifications

• Bachelor’s degree in Nursing required.
• Minimum of two (2) years of experience in behavioral health, substance use disorder treatment, public health, community health, infectious disease, wound care, or other relevant healthcare settings.
• Current licensure as a Registered Nurse in the State of New Jersey.
• Commitment to providing non-judgmental, person-centered care.
• Experience working with individuals using substances, people living with HIV/AIDS, individuals living with Hepatitis C, sex workers, and individuals experiencing homelessness.
• Ability to build trust and engage individuals who may be ambivalent about healthcare.

Integrity House operates 16 licensed treatment programs across New Jersey as well as community outreach and prevention programs, and serves over 3,000 individuals every year.

Integrity House is seeking a Nurse Care Manager to provide low-threshold health services at the Integrity Wellness Center. Located in Newark, NJ, the Integrity Wellness Center provides a welcoming, stigma-free environment for people who use drugs, sex workers, and people living with HIV/AIDS and/or Hepatitis C to access health and wellness resources and support. The center offers risk reduction supplies and education, health services, peer support, care coordination, laundry and personal care services, and other resources at no cost.

The Nurse Care Manager delivers basic health assessments, triage and stabilization services, wound care, infectious disease testing and prevention services, health education, and linkage to additional medical services. This is a hands‑on clinical role for a nurse who is comfortable providing low-barrier health services and building trust with individuals who may have had limited or negative experiences in traditional healthcare settings.

Key Responsibilities

• Service Delivery
  • Provide care in accordance with applicable nursing standards, standing orders, clinical protocols, and scope of practice.
  • Engage participants in non-judgmental conversations about urgent health concerns and long-term wellness.
  • Conduct basic health assessments and provide low-threshold clinical services for non-emergent health concerns.
  • Provide triage and stabilization for urgent or emerging health concerns and facilitate linkage to urgent or emergency care when needed.
  • Perform onsite care for wounds, abscesses, injection-related injuries, rashes, fungal infections, burns, and other skin conditions.
  • Provide education on overdose prevention, infectious disease prevention, wound care, medication adherence, and other relevant health topics.
  • Conduct rapid HIV and HCV testing and STI screening and support follow-up, including confirmatory testing and linkage to care.
  • Provide pre- and post-test education and counseling related to HIV and HCV testing.
  • Administer approved vaccines and support participant education, dose tracking, and follow-up.
  • Assist in overdose recognition, reversal, and monitoring as needed.
  • Coordinate referrals and warm handoffs to internal and external providers, including primary care, specialty medical care, HIV/HCV/STI treatment, and dental care.
  • Provide follow-up support when appropriate and welcomed by the participant.
  • Participate in outreach activities as needed.
  • Collaborate with team members to support coordinated service delivery.
• Documentation and Compliance
  • Review and implement applicable Harm Reduction Health standing orders, clinical protocols, and procedures in coordination with the Advanced Practice Nurse and program leadership.
  • Complete adverse event reporting requirements established by the New Jersey Department of Health.
  • Complete required Harm Reduction Health data collection and reporting, and maintain accurate clinical documentation in accordance with program, agency, state, and federal requirements.
  • Maintain nursing supplies inventory by monitoring usage, expiration dates, storage requirements, and replenishment needs.
  • Support appropriate storage, handling, and disposal of medications, vaccines, sharps, biohazardous materials, and medical waste.
  • Contribute to a safe, clean, and productive environment by complying with relevant agency policies and protocols, and requirements established by the New Jersey Department of Health and other regulatory bodies.
• Other Responsibilities
  • Complete required Harm Reduction Health orientation, training, and preceptorship.
  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies.
  • Support program evaluation, continuous quality improvement, and service development activities.
  • Participate in supervision, team meetings, and training activities.
  • Perform other duties as assigned.
