Prosano Health Solutions is growing its Advanced Primary Care team in partnership with Blue Cross Blue Shield of Arizona. The Clinical Care Guide will be a key contributor to our first-class care team, delivering end-to-end referral and testing navigation while collaborating with the care team to support patients toward their best health.

The role emphasizes patient care excellence, privacy, and compliance in a dynamic health setting, with real-time education and scheduling support for referrals

Qualifications

• Excellent interpersonal skills to interact with patients and team members
• Basic Life Saving (BLS) certification required
• High school diploma or GED required
• Interest in pursuing further education/certification
• Electronic Medical Records experience required
• Customer service orientation and experience essential
• Strong communication across modalities
• Highly collaborative and empathetic
• Must maintain confidentiality at all times
• Active AZ fingerprint clearance card required

Responsibilities

• Be a key contributor to the first-class care team.
• End-to-end responsibility for specialist and diagnostic testing navigation.
• Lead with the care team to support patients’ journey toward their best health.
• Emphasize customer/patient care excellence, privacy, and compliance.
• Support members in-person and virtually with referrals and follow-up.
• Contribute to improved health outcomes and cost savings.
• Be an empathetic problem-solver and visible patient partner.
• Explain referrals for specialist and diagnostic services.
