Job summary

ProviDRs Care in Wichita, KS is hiring a Medical Assistant – Care Navigator to join the Integrated Health Management team. This full-time on-site role supports member navigation, outreach, and care coordination within a health plan environment.

You will work with RN Case Managers, Utilization Review Nurses, and network staff to improve member access to services, coordinate DME, and report on program activity. Strong communication and organizational skills are essential.

Qualifications

• High school diploma or equivalent required.
• Completion of a Medical Assistant training program.
• Knowledge of basic medical terminology.

Responsibilities

• Assist with member outreach and care coordination.
• Gather demographic, medical, and provider information for RN Case Manager.
• Educate members about the Case Management program and resources.
• Coordinate follow-up with RN Case Manager and providers.
• Support care coordination and data collection across programs.
• Identify barriers to care and escalate as needed.
