Position: Care Navigator
Supervisor: Christan Holt
Schedule: Generally, Monday - Friday 8 AM-5 PM. Schedule subject to change as indicated by manager. (This is a hybrid position)
The Care Navigatorwill work closely with a Clinical Pharmacist Practitioner and Coordinating Registered Nurse and primary care teams at the practices to care for our patients. As a Care Navigator, you will serve as a patient advocate and be a liaison to help manage and improve patient outcomes and support the CM team with administrative and clinical tasks. An ideal candidate is a critical-thinker who is highly organized, patient-centric, and self-driven. Our Care Navigators are on the phone with patients throughout the day. They know how to build relationships by asking thoughtful questions and utilizing effective listening skills. They are responsible for clearly documenting patient interactions within Electronic Medical Record Systems (EMR) and company documents, so strong interpersonal, computer, and communication skills are needed. This position requires confidentiality, discretion, and exceptional customer service.
Key Responsibilities:
- Management of patient relationships in collaboration with clinical team for Enhanced Care Management, including frequent telephone interaction with patients
- Monitoring responses from patients, Care Management Care Navigators, providers
- Responding to or escalating RPM alerts within 2 hours of receiving
- Maintenance of therapeutic goal records for chronic care patients.
- Other associated tasks that may arise from time to time at the instruction of the Clinical Pharmacist Practitioner
- Call patients for appointment reminders and follow-up assessments.
- Contact patients for TOC calls and medication reconciliation in accordance with Value Based Care metrics
- Schedule patients for TMC visits
- Provides resources and data to patients for any questions or SDOH concerns that may arise during calls.
- Record patient's primary and secondary pharmacy insurance.
- Assessment of potential coupons, voucher, or other discounts available to the patient for medications.
- Communicating with Manufacturer’s medication assistance programs on behalf of the patient and/or provider
- Coordinate medication alignment as instructed by pharmacist as well as preparation of dispensing history.
- Contact patients and manufacturer programs for medication assistance.
- Aid Value Based Care Quality Team in their work to close HEDIS Metric gaps such as Medication Adherence metrics.
- Monitoring for and notification of emergent or new prescriptions
- Utilization and updating records in MedData software for MAP program
- Maintain a positive attitude and connect with patients.

- Follows HIPAA guidelines for patient confidentiality and maintains security of Protected Health Information (PHI) and business information.
- Does not exhibit, divulge, copy or remove the contents of any record or report except to fulfill work assignments.

PERFORMANCE REQUIREMENTS:
Knowledge, Skills & Abilities
Knowledge of medical terminology to communicate with clinical staff. Skill in preparing written detailed documentation and input data into computer system. Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to read and understand oral and written instructions. Ability to follow written protocols. Ability to work independently. Ability to communicate clearly.
Education:
- Medical Assistants: Graduate of an ABHES (Accrediting Bureau of Health Education Schools) or CAAHEP (Commission on Accreditation of Allied Health Education Programs) accredited program for Medical.
- Licensed Practical Nurse: Graduate of an accredited associate degree or bachelor’s degree LPN program.
Experience: 3-5 years preferred. Certified Health Coach preferred. An equivalent combination of education and experience may be considered.
Certificate/License:
- Medical Assistants: Certification from the AAMA (American Association of Medical Assistants) or Registration from the AMT (American Medical Technologists). Eagle does not accept certification or registration from any other entity.
- LPN: Unrestricted North Carolina Nursing License
