Appalachian Regional Healthcare (ARH) in Beckley, WV is seeking a Care Manager to coordinate patient management across the continuum of care. The position requires nursing licensure (LPN required; RN preferred) with 4–6 years of nursing exp.

Qualifications

• Associates Degree in Nursing (RN) licensed in state.
• Bachelors degree in Nursing preferred; must be obtained within 5 years.
• 4–6 years nursing experience preferred.
• LPN license required; RN preferred.

Responsibilities

• Manages an assigned caseload of patients from preadmission to discharge.
• Assumes responsibility for admission appropriateness and medical record monitoring.
• Coordinates communication with third party payers and the Utilization Committee.
• Identify managed care issues and address promptly as related to denials management.
• Coordinates with physicians, multidisciplinary team and other health care professionals.
• Revise and adjust plan of care daily based on ongoing assessment.
• Lead discharge planning meetings and document outcomes.
• Advocate for patient/family with respect to consent and advance directives.
• Maintain awareness of case management developments and seek ongoing education.
• Ensure continuity of care through discharge planning and follow-up.
