Job summary

Adagio Health seeks a Care Navigator to connect patients and community members to essential care, addressing social determinants of health and guiding enrollment in health programs. You will bridge clinical services with community support and coordinate care across nutrition and outreach initiatives.

The role requires CHW certification, strong communication, and the ability to travel regionally to partner sites.

Qualifications

• Accredited degree in Social Work or Public Health or related field.
• Active Community Health Worker (CHW) certification.
• Excellent interpersonal, verbal, and written communication skills.
• Proficiency with Microsoft Office and database systems.
• Experience in case management, care coordination, or care planning.
• Experience navigating enrollment processes for medical insurance (Pennie, Medicaid, CHIP).
• Ability to travel regionally for outreach and partner site visits.

Responsibilities

• Assess client and patient needs related to health, wellness, and social determinants of health (SDOH). Identify barriers and connect individuals with internal programs and partners.
• Provide warm handoffs and serve as a dedicated contact for seamless transitions between clinical appointments, nutrition support, and community services.
• Help community members navigate and enroll in health insurance (Pennie) and support WIC enrollment and intake.
• Build and maintain relationships with local service organizations and community partners to increase awareness and access to services.
• Deliver practical health education and share vetted resources to empower informed healthcare decisions.
• Collaborate with Clinical Services, Disease Prevention, and Nutrition & Education teams to coordinate holistic, person-centered care.
• Maintain accurate documentation of all patient interactions, referrals, and outcomes in data management systems.
