Rochester Regional Health System seeks a Care Navigator to join the Integrated Care team at St. Mary’s Campus, coordinating mental health care across PCPs, BHCs, and psychiatric providers. The role includes patient education, referrals, and supporting treatment plans within and beyond the clinic.
The position reports to the Manager, Behavioral Health/Primary Care Integration. Over-nights (7:00PM-7:00AM) on the St. Mary’s Campus, with a focus on engagement and outcomes in a collaborative setting.

The Care Navigator functions as a core member of an Integrated Care team that includes the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider. The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic. The Care Navigator may provide evidence-based treatments to patients or collaborate with other Mental Health providers when such treatment is indicated. This role will report to the Manager, Behavioral Health/Primary Care Integration or designee.

Sedentary - Sedentary roles are primarily office-based and require prolonged sitting (67-100% of the workday) with minimal physical effort and lifting limited to under 10 lbs. Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver’s license. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE: $20.00 - $26.00 CITY: Rochester POSTAL CODE: 14611 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
