Rochester Regional Health is seeking a Care Navigator to join Clifton Springs Hospital’s Addiction Services. You will coordinate mental health care within an Integrated Care team, coordinating referrals and supporting evidence-based treatments as indicated.
The role reports to the Manager, Behavioral Health/Primary Care Integration. Minimum qualifications include 2 years in human services or an Associate’s degree; a bachelor’s degree is preferred for advancement.

Summary: The Care Navigator functions as a core member of an Integrated Care team that includes the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider. The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic. The Care Navigator may provide evidence-based treatments to patients or collaborate with other Mental Health providers when such treatment is indicated. This role will report to the Manager, Behavioral Health/Primary Care Integration or designee.
