Job Summary

This new position requires an experienced project manager to develop and manage a clinical and research intake system with integrated care navigation for individuals with bipolar disorder who are seeking care through the Psychiatry Department at the University of Michigan. This position will interface with the ambulatory clinical intake call center, the general adult psychiatry intake team, the Bipolar Disorder Clinic and the Prechter Bipolar Disorder Research Program. The purpose of this position is to ensure efficient use of the resources of the Bipolar Clinic, coordinate effective screening of referrals and organize clinical information to enable the decision process regarding the suitability of the referral to the clinic and/or research. This position will also act as a care navigator for patients, assisting patients in movement through the system. The Project Manager will interface with community-based providers in the context of a collaborative care model that will include facilitating referrals to the community and offering consultations to community providers.

Responsibilities*

• Management and leadership in Clinical Screening, Navigation, Triage, and Support
• Identify and develop appropriate pathways for individual patients through the clinical and research systems, include work plans, schedules, resource plans and status reports.
• Facilitate Michigan Medicine clinic appointments and address any barriers to care, striving to ensure continuity and quality of treatment.
• Assist and support patients through the appointment process.
• Facilitate or provide referrals outside of Michigan Medicine when appropriate.
• Oversee and supervise screening of all referrals to the Bipolar Clinic and Prechter Bipolar Research Program.
• Provide information about ongoing research studies in bipolar disorder and facilitate contact with research teams for more information.
• Simplify complex medical information into language patients, families and caregivers can understand and apply.
• Care Coordination and Multidisciplinary Collaboration
• Establish and maintain positive working relationships with key members of the healthcare team and community resource groups.
• Manage workflow process for two-way referrals from multiple services.
• Develop and revise screening materials specific to the Bipolar Clinic
• Serve as a liaison between the Bipolar Clinic, support services and community resources to facilitate access to additional support for patients.
• Stay updated on the latest research, treatment guidelines and best practices in bipolar disorder management.
• Patient Education and Outreach
• Identify patient and family learning needs and ensure they are met.
• Provide support, education and resources to patients and their families to enhance their understanding of bipolar disorder, treatment options, medication adherence, coping
• strategies and community support services.
• Develop educational materials, brochures and resources for patients, families and the community, promoting mental health literacy and reducing stigma surrounding bipolar disorder.
• Documentation and Data Management
• Maintain accurate and up-to-date documentation of patient interactions and care coordination activities within the EHR.
• Ensure compliance with privacy regulations and ethical standards in handling sensitive patient information.
• Input, retrieve and interpret data from computer-based information systems.
• Participate in the tracking and monitoring of metrics and outcomes, in collaboration with clinical administration, to document and evaluate program outcomes.
• Collaborate with research teams within the Prechter Bipolar Research Program and related projects such as the Learning Health Networks for bipolar disorder, to collect relevant data for research purposes, ensuring confidentiality and adherence to research protocols.

Required Qualifications*

• Master's degree in a relevant field such as psychology, social work, nursing, or counseling.
• Prior experience working with individuals diagnosed with bipolar disorder or a related mental health condition. Typically requires more than four years of experience.
• Strong knowledge of bipolar disorder, including its symptoms and diagnostic criteria.
• Excellent interpersonal and communication skills to effectively engage with patients and families.
• Demonstrated abilities in interpersonal, verbal and written communication skills and problem-solving skills within a health care system.
• Strong organizational skills and attention to detail in documentation and data management.
• Experience with supervision of employees.
• Knowledge of research methodologies and the ability to contribute to quality improvement initiatives or research projects.
• Ability to work independently, prioritize tasks, and manage diverse tasks effectively.
• Flexibility to adapt to the evolving needs of the Bipolar Clinic and to collaborate with a multidisciplinary team.

Desired Qualifications*

• Proficiency in using electronic health record systems and relevant software applications.
• Proficiency in care coordination, including scheduling appointments, coordinating referrals and collaborating with multidisciplinary teams.
• Knowledge of community resources, support services and relevant mental health organizations.
