Job summary

Molina Healthcare in Wisconsin seeks a Care Coordinator to support waiver services and member care planning. You will conduct assessments, facilitate enrollment and disenrollment, and develop coordinated care plans with members, families, and providers.

You will monitor progress, authorize services, and ensure integration of behavioral health, LTSS, and community resources. A valid driver’s license and ability to travel 25–40% are required.

Qualifications

• At least 2 years of health care experience, including 1 year in LTSS/care management or related setting.
• Clinical licensure and/or certification required only if state contract mandates it.
• Bachelor’s degree in healthcare may be required depending on state/contract.
• Valid driver’s license and reliable transportation for travel requirements.
• Experience working with persons with disabilities/chronic conditions is preferred.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians, and support network.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Promotes integration of behavioral health, LTSS, and community resources for continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Collaborates with ICT meetings for service approvals/denials.
• Uses motivational interviewing and Molina clinical guideposts during member contacts.
• Identifies barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
• Develops prevention plans for member health and welfare.
• Travel 25–40% may be required.
