Job summary

Mount Sinai Medical Center is seeking a Care Manager to identify patients needing care management, gather and prioritize information, and coordinate discharge planning with families and the care team. You will document assessments, conduct reviews, and ensure smooth transitions of care using InterQual criteria.

The role emphasizes collaboration with physicians, social workers, and the interdisciplinary team to optimize outcomes and minimize readmissions.

Qualifications

• RN license (FL) required or MD for FMG.
• 2+ years as a Care Manager in a hospital setting.

Responsibilities

• Proactively identifies patients in need of care management services.
• Gathers information about populations and prioritizes workload.
• Responds to referrals and offers alternatives when appropriate.
• Documents admissions assessments and utilization reviews.
• Develops and documents discharge plans with patient/family and interdisciplinary team.
• Communicates outcomes and uses InterQual criteria.
• Coordinates transitions of care and post-acute referrals.
• Keeps patient-centered communication and participates in weekend/on-call rotation.
