Job summary

Mount Sinai Medical Center is seeking a Registered Nurse with care management expertise to proactively identify patients in need, gather population data, and coordinate interventions. The role emphasizes timely referrals, comprehensive assessments, and efficient discharge planning.

The candidate will work within a multidisciplinary team to address barriers to care, ensure safe transitions, and reduce readmissions, upholding high standards of patient-centered care in a large Florida hospital

Qualifications

• Current RN FL license is required for nurses or a Medical Doctor's degree for Foreign Medical Graduates (FMG).
• Education Bachelor's Degree in Nursing preferred or proof of Medical Doctors Degree required for FMG.
• Experience Proof of RN licensure, Foreign Medical Graduate (FMG), or at least 2 years as a Care Manager in a hospital is required.

Responsibilities

• Proactively identifies patients in need of care management services/intervention.
• Gathers information about assigned populations and prioritizes workload to meet outcomes.
• Responds to referral requests in a timely manner and offers alternatives if referral isn't appropriate.
• Conducts admissions assessments in accordance with department standards.
• Conducts utilization reviews in accordance with department standards.
• Maintains a holistic view of the patient and their needs.
• Develops and documents a plan to address barriers to care with patient/family and team.
• Documents discharge plan and updates as patient condition changes.
• Addresses issues that may put a patient at risk for readmission.
• Communicates transitions of care with patients and families.
