Job summary

Qualifications

• Washington RN license required upon hire.
• Bachelor's Degree in Nursing preferred.
• Three years current experience in a health care environment with home health case management experience.

Responsibilities

• Evaluate and manage the changing health and service needs of the patient/family unit using the nursing process and interdisciplinary team.
• Collaborate with the patient’s Primary Care Physician and health team to create and implement a written care plan within documentation standards.
