Qualifications

• Associate's Degree in Nursing is required for RN candidates.
• Master's Degree in Social Work is required for LICSW candidates.
• Minimum of 3 years of independent clinical practice as an RN or LICSW.
• 1 year of case management experience for Medicaid populations preferred.

Responsibilities

• Assess members through face-to-face or telephone calls to determine care coordination needs.
• Complete a comprehensive person-centered assessment including health history and supportive needs.
• Coordinate physical, behavioral health, and social services.
• Provide medication management and support medication adherence.
• Identify problems/barriers for care coordination and interventions.
• Create a plan of care to help members achieve optimal health.
• Identify goals with time frames and priorities for completion.
• Schedule follow-up calls and document progress toward goals.
• Coordinate care with Care Coordinators and other providers.
