About This Role

The In-Office Care Manager (RN/LICSW) assists members in achieving their optimal level of health through self-management and care coordination services. This role is ideal for experienced professionals who are passionate about engaging with members and advocating for their health needs.

• **Schedule:** Full-time, Monday through Friday, 8:30 AM - 5:00 PM EST (4 days in-office, 1 day remote)

What You'll Do

• Assess members through face-to-face or telephone calls to determine care coordination and management needs.
• Complete a comprehensive person-centered assessment including health history and supportive needs.
• Coordinate physical, behavioral health, and social services.
• Provide medication management and support medication adherence.
• Identify problems/barriers for care coordination and appropriate interventions.
• Create a plan of care to assist members in achieving optimal health.
• Identify goals and assign priority with associated time frames for completion.
• Schedule follow-up calls as necessary and document progress towards goals.
• Coordinate care and services with Care Coordinator, Community Health Navigator, and other providers.

Requirements

Education:

• Associate's Degree in Nursing required for RN candidates.
• Master's Degree in Social Work required for LICSW candidates.

Experience:

• Minimum of 3 years of independent clinical practice experience as an RN or LICSW.
• 1 year of case management experience supporting complex adult and pediatric Medicaid populations preferred.

Licenses & Certifications:

• Active and unencumbered Registered Nurse (RN) licensure in the District of Columbia.
• Active and unencumbered Licensed Independent Clinical Social Worker (LICSW) licensure in the District of Columbia.
