Qualifications

• Bachelor's Degree in Nursing required or higher within 3 years if no Bachelor's degree is currently held.
• California RN license required at hire.
• California Fire and Life Safety Card required within 30 days of hire.
• 2 years experience in healthcare field (acute, ambulatory, post-acute, etc.).
• 2 years experience in Case Management or successful completion of the Transitions in Practice (TIP) program for Care Manager.

Responsibilities

• Coordinate care across patients, caregivers, nurses, social workers and physicians to ensure effective transitions in care and utilization management.
• Engage in collaborative case management practices to optimize patient outcomes and resource use.
