Job summary

Providence Health & Services is seeking a Care Manager RN for a full-time day shift to lead care management efforts for at‑risk patients in an acute care setting.

The role emphasizes assessment, planning, coordination of care, discharge planning, and strict adherence to regulatory standards while advocating for patients throughout the continuum of care.

Qualifications

• Associate degree in nursing required at hire.
• California RN license required.
• California Fire and Life Safety Card due within 30 days of hire.
• 2 years acute-care experience in medical-surgical or inpatient setting.
• Successful completion of TIPS program or Case Management Orientation Program upon hire.

Responsibilities

• Provide professional, comprehensive care management for at‑risk patients in an acute care environment.
• Assess and plan patient care, coordinate care, and manage resource utilization and discharge planning.
• Document interventions and ensure regulatory compliance while advocating for patients across the continuum of care.

Education

Associate degree in Nursing

2 years acute-care experience (medical-surgical or inpatient)

TIPS/Case Management Orientation Program

Care Manager RN (Full‑time Day Shift)

The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient‑centric care management services for at‑risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal is to ensure appropriate healthcare resources are used throughout the continuum, so the care provided is the right care, at the right time, in the right setting.

Required Qualifications

• Associate’s Degree in Nursing upon hire
• California Registered Nurse License upon hire
• California Fire and Life Safety Card (National Organization) within 30 days of hire
• 2 years acute‑care experience in a medical surgical or inpatient setting
• Successful completion of TIPS program or Case Management Orientation Program upon hire

Preferred Qualifications

• Bachelor’s Degree in Nursing or higher within 3 years of hire
• Case management certification (ACM or CCM)
