Providence Medical Foundation is seeking a skilled nurse to join our care management team, collaborating with patients, nurses, social workers and physicians to coordinate care across settings.
The ideal candidate is a California-licensed RN with 3 years of clinical experience in an HMO or medical group; preferred qualifications include Bachelor's or Master's in nursing and CCM/ACM certification. We offer strong benefits and a mission-driven culture.
Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination.

Graduate of an accredited school of nursing.
California Registered Nurse License upon hire.
3 years Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.
Bachelor’s Degree in Nursing, Or
Master's Degree in related field.
Certification in Case Management (CCM, ACM) upon hire.
Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire.
3 years experience in utilization management or case management.
