Providence Medical Foundation is seeking a qualified registered nurse to join our care management team. The role emphasizes collaborative practice with patients, nurses, social workers, physicians, and the community to coordinate care across the continuum.
The successful candidate will hold a California RN license, graduate from an accredited nursing program, and bring at least 3 years of clinical experience in an HMO, medical group, hospital, or clinic setting.
Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination.

Graduate of an accredited school of nursing.
California Registered Nurse License upon hire.
3 years Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.
Bachelor’s Degree in Nursing, Or
Master's Degree in related field.
Certification in Case Management (CCM, ACM) upon hire.
Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire.
3 years experience in utilization management or case management.
