Job summary

Providence is seeking a Care Manager to coordinates the full spectrum of patient care across inpatient and outpatient settings. You will support physicians in management and coordination of care and work across multiple clinic and provider locations to ensure cost-effective utilization of resources.

The role requires a Washington RN license at hire, with preference for a Bachelor's in Nursing, Certified Case Manager credentials, and 2 years of clinical experience in utilization review or case

Qualifications

• Washington Registered Nurse License required at hire.
• Bachelor's degree in nursing preferred.
• Certified Case Manager preferred.
• Two years of clinical experience.
• Experience in utilization review / case management preferred.

Responsibilities

Job description

Coordinates care for Pacific Medical Centers' patients (excluding USFHP) in order to assure cost effective, appropriate utilization of health care resource. Care Manager manages, directs, and coordinates the entire spectrum of care for patient throughout a variety of settings, both inpatient and outpatient. This position assists physicians (internal and external) with management and coordination of care. Works at varied clinic and provider locations as needed.
