Job summary

Pacific Medical Centers (PacMed) is seeking a Care Manager to coordinate the comprehensive care for patients across inpatient and outpatient settings. You will support physicians in managing and coordinating care and may work at multiple clinic and provider locations as needed.

Washington RN license is required at hire, with preferred Bachelor's in nursing and experience in utilization review/case management to enhance patient outcomes within Providence-affiliated care networks.

Job description

Coordinates care for Pacific Medical Centers' patients (excluding USFHP) in order to assure cost effective, appropriate utilization of health care resource. Care Manager manages, directs, and coordinates the entire spectrum of care for patient throughout a variety of settings, both inpatient and outpatient. This position assists physicians (internal and external) with management and coordination of care. Works at varied clinic and provider locations as needed.

Required Qualifications

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Pay Range: $52.59 - $83.03
