Job summary

Description

Coordinates care for Pacific Medical Centers' patients (excluding USFHP) in order to assure cost effective, appropriate utilization of health care resource. Care Manager manages, directs, and coordinates the entire spectrum of care for patient throughout a variety of settings, both inpatient and outpatient. This position assists physicians (internal and external) with management and coordination of care. Works at varied clinic and provider locations as needed.

Required Qualifications:
  • Upon hire: Washington Registered Nurse License
Preferred Qualifications:
  • Bachelor's degree in nursing
  • Upon hire: Certified Case Manager
  • 2 years Clinical experience
