Job summary

Providence Centralia Hospital seeks an Inpatient Registered Nurse (RN) Care Manager to deliver comprehensive, patient-centric care management in an acute care setting. Responsibilities include assessment, planning, care coordination, discharge planning, and advocacy to ensure appropriate resources and timely care across the continuum.

Washington RN license and 2 years of acute care experience required; 1 year in care management or utilization review, or completion of TIPS/Case Management

Qualifications

• Associate's Degree in Nursing and Washington RN license required.
• Minimum 2 years of acute care experience in a Medical-Surgical or inpatient setting.
• 1 year of care management or utilization review experience, or completion of TIPS/Case Management Orientation Program.

Responsibilities

• Assess and plan patient care in an acute care environment.
• Coordinate care and discharge planning across settings.
• Monitor resource utilization and care coordination throughout the continuum.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and ensure appropriate setting and timely care delivery.

Job description

The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management RN is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting.

Practices in accordance with the Nurse Practice Act in the state of employment, as well as the standards of practice as outlined by the American Case Management Association Ensures ongoing professional development by remaining current with best practices, industry standards and regulatory requirements.

Required Qualifications:

• Associate's Degree in Nursing degree/diploma upon hire
• Washington Registered Nurse License upon hire.
• 2 years of Acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review in any setting or upon hire successful completion of TIPS program or Case Management Orientation Program.

Preferred Qualifications:

• Bachelor's Degree in Nursing or higher within 3 years of hire
• National Certification in area of specialty upon hire
