Qualifications

  • RN license in state of practice is required.
  • Associates or diploma in nursing required; bachelor’s preferred.
  • 2 years of clinical experience required; case management experience preferred.
  • BLS certification required within 60 days of hire.

Responsibilities

  • Collaborate with patient/family, social workers, physicians, and the payer to coordinate care.
  • Coordinate discharge planning to improve patient flow and length of stay.
  • Assess, plan, implement and evaluate care needs for NICU patients.
  • Monitor cost-effective care and ensure regulatory compliance and patient safety.

Pay Range:$35.92 - $53.88A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.Hospital Sisters Health System (HSHS) is seeking a Care Manager-RN to join our NICU team. In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting.
