Job Description
Job Summary:

Start Date: Monday June 16th, 2025

Salary: $85,000 annually, plus comprehensive benefits package

The Care Manager supports the implementation of the Value Based Care Management program by providing high-quality telephonic Case or Care Management with CareFirst members. The Care Manager partners with members, caregivers, providers, and the interdisciplinary care team to ensure effective care plans and positive member experiences that improve health outcomes. Responsibilities include advocacy, motivational interviewing, complex case management, and supporting members throughout their care plans.

Essential Job Functions
  1. Engage telephonically with members, caregivers, and providers to develop comprehensive care plans and address member needs.
  2. Collaborate with healthcare providers to meet health goals through care plan development and implementation.
  3. Assess ongoing care needs, monitor progress, and revise plans as necessary to reflect changes in condition or preferences.
  4. Coordinate patient education using appropriate modalities.
  5. Identify benefits and community resources, evaluate social determinants of health, and facilitate referrals.
  6. Assist with coordination of tests, imaging, and specialist consultations.
  7. Perform medication reconciliation regularly and assess for efficacy and interactions.
  8. Facilitate and monitor transitions of care, including post-discharge follow-up.
  9. Maintain quality documentation of care plans and progress.
  10. Follow Value Based Care Management Program guidelines and meet productivity and quality metrics.
  11. Complete mandatory training and participate in team learning activities.
Specific Skills/Attributes
  • Strong motivational interviewing and case management skills.
  • Self-directed, organized, problem-solving abilities, and ability to meet deadlines.
  • Excellent communication and customer service skills.
  • Ability to work effectively in a fast-paced environment and engage members effectively.
  • Proficiency in data analysis and medical documentation.
  • Experience with care plan documentation and working within organizational structures.
Qualifications
  • Current multi-state compact RN license; BSN preferred.
  • Training in motivational interviewing preferred.
  • 3-5 years of clinical experience, telephonic case management preferred.
  • Competency with electronic medical records and Microsoft Office tools.
  • Home office that meets HIPAA and internet requirements.
  • Effective communication skills for interacting with members and healthcare professionals.
