Job summary

Providence Swedish Medical Center in Olympia, Washington is seeking a Care Coordinator to promote coordinated and continuous care for complex patients. The ideal candidate will have a Master’s Degree in Social Work and experience in health care coordination.

Qualifications

• 3 years of social services experience with a healthcare related organization.

Under supervision of Care Management Manager, works as a catalyst to promote coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre‑admission to immediate post‑discharge.

Required Qualifications

• Master's Degree in Social Work or related field
• Upon hire: Washington Clinical Independent Social Worker Associate License
• 3 years of social services experience with a health‑care related organization; experience should include a minimum of 2 years in acute care/care coordination.

Preferred Qualifications

• Upon hire: Accredited Case Manager (ACM)
• Upon hire: Certified Case Manager (CCM)
