Job summary

Molina Healthcare is seeking a Care Manager to support vice teams in coordinating long-term services and supports (LTSS) for members with high-need potential. You will collaborate with a multidisciplinary team to ensure outcomes and cost-effective care across the continuum.

Responsibilities include assessing needs, developing care plans, monitoring progress, and promoting integration of behavioral health and LTSS resources. Requires travel up to 40% as needed by state contracts.

Qualifications

• At least 2 years of health care experience incl. LTSS and care management.
• Clinical licensure/certification required only if state contract/regulation requires it.
• Bachelor-level health care degree may be required by state contracts.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements waiver service plans with members, caregivers and providers.
