Longview Regional Medical Center is seeking a Licensed Master Social Worker (LSW-LCSW/LMSW) to coordinate discharge planning, transitions of care, and case management. You will collaborate with interdisciplinary teams to develop discharge plans, assess post-acute needs, and ensure compliance with regulatory standards.

Qualifications

• Requires Bachelor’s Degree in Social Work (BSW).
• MSW preferred; active licensure encouraged.
• 2–4 years in an acute care hospital setting.

Responsibilities

• Coordinates discharge planning and care transitions with IDT.
• Reviews medical records for post-acute care needs.
• Maintains compliant documentation of case management activities.
• Serves as liaison with community agencies for patient transitions.
• Assists with care planning and interdisciplinary meetings.
• Adheres to federal, state, and Joint Commission standards.
