• Conduct initial assessments, ongoing evaluations, and ongoing care planning to ensure the highest level of care and support
• Collaborate with healthcare providers, family members, and other care team members to ensure seamless care coordination
• Monitor and report on client progress, identifying areas for improvement and implementing changes as needed
• Provide education and support to clients and their caregivers on disease management, medication, and overall health
• Manage and supervise care teams, ensuring that all team members are equipped to provide the highest level of care
• Maintain accurate and timely records of client care and services
