A First Name Basis Home Care is seeking a dedicated Care Manager to oversee individualized care plans and coordinate services for clients in the Houma area. You will supervise staff, assess client needs, and collaborate with community resources to ensure high-quality care and client well-being.

Requirements include 2+ years in homecare, a high school diploma (Bachelor’s preferred), excellent communication, and reliable transportation for area travel.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor's degree preferred or equivalent work experience.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
• Supervise and mentor staff, fostering a collaborative environment that promotes professional growth.
• Conduct assessments to identify client needs and coordinate appropriate services.
• Collaborate with community resources and stakeholders to enhance service delivery.
• Maintain accurate documentation of client progress and interventions in accordance with organizational policies.
