A First Name Basis Home Care is seeking a dedicated Care Manager to support clients and coordinate services across our local network. You will develop personalized care plans, supervise staff, and connect clients with community resources to enhance well-being.

Ideal candidates have 2+ years in homecare, a high school diploma (bachelor's preferred), strong communication, and reliable transportation to travel within the service area. Competitive pay and comprehensive benefits are offered.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor's degree preferred or equivalent work experience.
• Office/administrative support skills.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
• Supervise and mentor staff members, fostering a collaborative environment that promotes professional growth.
• Conduct assessments to identify client needs and coordinate appropriate services.
• Collaborate with community resources and stakeholders to enhance service delivery.
