A First Name Basis Home Care is seeking a dedicated Care Manager to develop personalized care plans and ensure client well-being through effective supervision and services coordination. The role focuses on assessing needs, coordinating resources, and maintaining detailed documentation to ensure high quality care.

You will lead and mentor caregivers, collaborate with community partners, and drive continuous improvement in care delivery.

Qualifications

• 2+ years of experience in homecare (qualifications for the role)
• High school diploma required; Bachelor's degree preferred or equivalent work experience
• Office/administrative support skills
• Excellent written and verbal communication skills
• Reliable vehicle for travel within area
• Ability to work in a fast paced, multi-task environment with competing priorities

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
• Supervise and mentor staff members, fostering a collaborative environment that promotes professional growth.
